If you received a Sacrament at St. Joseph’s Parish and require a copy of a Sacramental Certificate (Baptism Confirmation, or Marriage), you are asked to complete a Sacrament Certificate Request Form and return it, along with a piece of photo ID, to the Parish Office. You can submit the completed form in person, by regular mail, by fax, or by email. There is an administrative fee of $20.00 for a certificate – payable by cash, cheque, or Visa/Master Card.

St. Joseph’s Parish can provide a Sacramental Certificate to the following:

• Yourself (provided valid photo ID)
• A parent/legal guardian can request a certificate for his/her child if it is for a minor
• If you have proof of guardianship, power of attorney or are an executor, you can request a certificate for that person
• You can also obtain a certificate for a deceased family member if you can prove relationship in addition to providing a death certificate
